Maiden name is misspelled and birthdate is incorrect. Birthdate is actually September 4, 1894. Otta was born in Evansville, Arkansas and died in Spiro, Oklahoma. She died at the age of 42 of Malaria. Her death certificate states 'Malaria. Died for want of proper medication.' Headstone was ordered by my uncle Lewis Flaherty a very long time ago. Otta was my Grandmother.
